Administrative Forms
Refund Request Form - A full refund will be issued any time prior to the end of registration. Once registration closes all refunds will be minus a $35 administrative fee. There will be no refunds once games begin or due to game cancelations, regardless of the cause. Credit card processing fees will not be refunded at any time.
Team Sponsorship Form - For teams who received sponsorship from corporate sponsors or local businesses, Please read the form for specific rules and guidelines of the Team Sponsorship program.
Volunteer Board Member Application - For volunteers interested in serving on an ASA Sport Board or Committee.

BASEBALL
Baseball Equipment Request Form - This form should be submitted by the head coach to request loaner equipment for the current season. Equipment bags are distributed at the Coaches Meeting.
Game Protest Form - Please read the local league regulations for complete information on how and when a protest can be filed.
BASKETBALL
Coach Schedule Conflict Form - Basketball Head Coach can submit schedule conflict requests with this form.
Score Sheet (K-Middle School) - Coaches should bring a completed scorebook sheet to each game during the season.
High School Score Sheet - Coaches should bring a completed scorebook sheet to each game during the season.
Officiating Evaluation Form - Use this form to submit feedback on basketball officials.
SOCCER
Academy Player Form - See Academy Soccer League Information.
New Soccer Player Birth Certificate Form - Use this form to upload a copy of a birth certificate for any new player to ASA Soccer.
ASA Soccer Athlete Scholarship Application - This application is for current high school seniors who have participated in ASA as a player for a minimum of 3 years (not seasons) or as referee for 1 year (not season). *Deadline for submission is 5:00 pm, April 19, 2024.
VOLLEYBALL
Practice Gym Request Form - The Head Coach must submit this form prior to the coaches meeting each season.
Equipment Rental Request - Rental requests must be submitted by current ASA coaches only.
